Dear Mr. Sapers:

I write to follow up on our recent discussions concerning the above mentioned matter, and to provide you with a formal list of commitments which I made to you during our conversation on March 7, 2011.  Please find attached a copy of the same. 

I have also attached for your reference a copy of our recent review into this matter.

As I mentioned to you during our discussion, I consider the gaps identified in the reports regarding this incident to be serious and requiring a comprehensive response as outlined in the attachment. The lack of managerial oversight and incomplete reporting at the local level is extremely concerning to me.
